CONSENT AND INDEMNITY

• I hereby state that in consideration of the care provided and facilities made available to our child at Islamic Worldview School (herein known as IWS), we hereby authorize and give our consent to IWS, its staff, and members to;

1. Seek medical treatment in the event of illness, accident or any emergency when neither of us nor the nominees can be contacted by phone. I further agree to meet all expenses incurred in connection with such treatment.

2. For our child's participation, organize all indoor and outdoor activities, including outings during our child's attendance at IWS. We understand that IWS will, as far as possible, provide the necessary care to ensure our child's safety. In this respect, we agree that we will not hold IWS and/or its employees responsible and liable for any claims due to injuries suffered by the child or mishap that may arise during our child's attendance at IWS.

3. We permit our child's photograph for school display purposes, IWS social media platforms, printed publications, and other promotional items.
