ACKNOWLEDGEMENT OF DEVELOPMENTAL / EDUCATIONAL NEEDS

CONSENT AND INDEMNITY

• I hereby state that in consideration of the care provided and facilities made available to our child at Islamic Worldview School (herein known as IWS), we hereby authorise and give our consent to IWS, its staff and members to;

1. Seek medical treatment in the event of illness, accident or any emergency when neither of us or nominees can be contacted by phone. I further agree to meet all expenses incurred in connection with such treatment.

2. Organise, for our child's participation, all indoor and outdoor activities including outings during the period our child's attendance at IWS. We understand that IWS will, as far as possible, provide the necessary care to ensure the safety of our child. In this respect, we agree that we will not hold IWS, and/or its employees responsible and liable for any claims due to injuries suffered by the child or mishap that may arise during the period of our child's attendance at IWS.

3. We permit our child's photograph to be used within school display purposes, IWS social media platforms, printed publications and other promotional items.
